Eternal offers two plans. The Healthspan plan costs $150 per month on a one-year contract, or $1,299 paid annually. The Concierge plan costs $600 per month on a one-year contract, or $6,000 per year. Both plans qualify for FSA and HSA funds.

Eternal does not bill insurance. The practice operates on a direct-pay model. Members pay membership fees directly and can use HSA or FSA funds. Eternal provides itemized receipts for members who want to submit expenses independently.

The Healthspan plan includes dedicated primary care with a nurse practitioner, a comprehensive annual exam, a DEXA scan, and a yearly performance assessment. That assessment covers 70-plus blood biomarkers and 90-plus health and performance biomarkers through VO2 max, lactate threshold, strength and power, and mobility and stability testing. Members receive a full health report and access to the Eternal iOS app.

The Concierge plan provides a dedicated MD instead of a nurse practitioner, plus twice-yearly assessments tracking 195-plus biomarkers. Members also receive a dedicated sports dietitian, a personal coach with bi-weekly check-ins, personalized health reports with care team video commentary, an aging and longevity strategy from an MD, and coordinated specialist referrals.

Eternal operates two performance centers. Eternal Presidio sits at 1183 Old Mason Street in San Francisco, California, at the base of the Golden Gate Bridge in the Presidio. Eternal Chelsea is at 525 West 26th Street in New York City, along the High Line in West Chelsea.

Yes, Eternal members use the Eternal app on iOS to schedule appointments, message their care team, request prescription refills, and meet their PCP virtually. The app also tracks health data, displays biomarker trends, and stores personalized health reports and goals.
