Yes. The practice offers quarterly payment by check or credit card. For credit card payments, the first charge processes at enrollment, and the practice automatically charges each subsequent quarter. If you prefer a check, the practice invoices you quarterly.

Yes. You can terminate your membership with 30 days of written notice. The practice refunds the unused portion of your annual fee on a pro-rated basis.

Members receive 24/7 access to Dr. Esper's personal cell phone. The practice offers same-day or next-day appointments with minimal to no wait time. During office hours, you can also reach the medical assistant directly by text, secure email, or phone. Dr. Esper can join specialist appointments by phone in complex medical situations.

Dr. Esper covers both. Members receive full concierge primary care alongside sleep medicine. She handles acute episodic care, chronic disease management, preventive medicine, and wellness visits, in addition to diagnosing and treating sleep disorders. Patients who want sleep services only may see her outside the concierge membership.

Yes, for covered services. The practice bills your insurance or Medicare for regular office visits and wellness visits. Your deductibles and copays still apply under your plan. The annual membership fee itself is a separate charge that insurance and Medicare do not cover.

Eligibility depends on your specific plan. The practice recommends consulting your FSA or HSA administrator, employer, HR representative, or tax advisor to clarify whether your plan qualifies.

During Dr. Esper's absences, a substitute physician covers her patients with the same level of availability. The practice provides instructions on how to reach the substitute doctor when needed.

Yes. Out-of-town visitors can schedule office appointments for acute care for up to two weeks per year. The practice bills their insurance in the normal course. Stays beyond two weeks are handled on a case-by-case basis.

Download the Patient Enrollment Form from the practice website and mail it to the address provided. Once the practice receives your form, it mails you a Patient Membership Agreement to sign and return. After receiving your signed agreement, the practice confirms your admission. If enrollment is full, the practice places you on a waiting list and notifies you of your status.
