The annual retainer is $2,400, or $200 per month. This fee covers a comprehensive history and one-hour annual physical as well as ongoing, as-needed medical care throughout the year. Rates have been in effect since October 14, 2019.

Yes. The yearly retainer covers most primary care services, but certain procedures and services fall outside the plan. Dr. Ball requires members to maintain primary insurance or Medicare to cover those additional needs.

The annual physical runs approximately one hour. Before the visit, patients submit prior medical records, a family history, and a three-day nutrition diary. The exam includes lab work such as a CMP, CBC, TSH, cholesterol panel, glucose, PSA, and urinalysis, along with a resting EKG, spirometry, bone density screening, and an ankle-brachial index screening.

Dr. Ball offers same-day and next-day appointments with minimal to no wait time. After-hours and weekend visits are also available for members who need care outside regular office hours.

Yes. Members have 24/7 direct access to Dr. Ball by personal cellphone, text, email, Skype, and FaceTime. Telephone consultations are also available as part of the membership.

Yes. Dr. Ball conducts telemedicine visits for members who prefer a remote option. Telephone consultations are also part of the standard membership.

Every member receives access to FitForImpact, Dr. Ball's online wellness program, as part of the retainer. The practice also provides a discounted gym membership through its partnership with Life Changing Fitness in Tyler.

Yes. Dr. Ball manages detailed specialist referrals and care coordination, including follow-up. He also provides pre-operative medical consultations and visits members in the hospital to advocate on their behalf with hospital staff and specialists.

The practice offers a comprehensive range of preventive services. These include cardiovascular health and heart attack and stroke prevention programs, vision and hearing testing, anxiety and depression screening, immunizations, mammogram coordination, colonoscopy coordination, skin biopsies, and cryotherapy for skin lesions.
