The practice charges no copays, no deductibles, and no additional fees beyond the monthly membership. Young adult dependents ages 19 to 24 can join for an additional $40 per month.

The practice accepts monthly credit card payments, quarterly payments, or annual payments. Automatic credit card payment plans are also available. Members choose whichever schedule works best for them.

Dr. Jacobsen advises Medicare patients to avoid HMO plans that restrict access to out-of-network providers.

Yes. The practice schedules same-day and next-day urgent appointments. Dr. Jacobsen also offers evening, weekend, and holiday availability. Members reach her directly by cell phone, text, email, or video chat around the clock.

The $199 monthly membership covers all services Dr. Jacobsen provides in the clinic. That includes EKGs, spirometry, in-house lab work, minor skin lesion removal, cryotherapy, laceration repair, splinting, casting, and ear wax removal. Members pay nothing extra for any of these services.

Yes to both. Dr. Jacobsen communicates with members via video chat, phone, text, and email. She also makes home and workplace visits. Members who are homebound or in nursing facilities should contact the practice to discuss their options.

The practice no longer accepts patients ages 2 to 18. Adults may add young adult dependents ages 19 to 24 to their membership for $40 per month.
