Bloom Direct Primary Care

Bloom Direct Primary Care charges $95 per month for adults ages 20 to 64. Children ages 0 to 19 pay $40 per month and must enroll with at least one parent or guardian. Adults 65 and older pay $125 per month. A one-time registration fee of $225 applies per member. Family and employer group discounts are available.

Bloom Direct Primary Care does not currently accept patients enrolled in Medicare. Prospective patients can contact the practice directly with questions about eligibility.
