Amity Health charges $110 per month for adults ages 18 to 65 and $125 per month for members 65 and older. Dependent children ages 0 to 26 with a parent enrolled pay $25 per month. A one-time $100 sign-up fee applies. Members who sign up for a full year in advance receive their first month free.

The membership covers 95% of primary care needs with no copays and no surprise bills. Members receive annual blood work including CBC, CMP, A1C, and lipid panels. The membership also includes 24/7 access to the doctor by call, text, video, or email, same or next day appointments, telehealth visits, and e-consults with specialists through Thea Health at no additional cost.

Amity Health does not accept insurance for membership fees. Members pay directly each month. For lab work, the practice partners with Quest Diagnostics to offer wholesale pricing. Members can also use insurance for imaging or additional lab work when applicable.

Yes. Members have 24/7 direct access to their doctor by call, text, video call, or email. The practice offers same or next day in-person appointments with no wait times, as well as telehealth appointments by voice or video for added convenience.

Yes. Amity Health offers home visits within a 10-mile radius of either location for $99. This option gives members flexibility when an in-office visit is difficult to arrange.

Yes. Amity Health provides PRP therapy, testosterone replacement therapy, weight management programs, IV therapy, Botox and aesthetic services, functional nutrition with GI-MAP analysis, genetic testing, and sports medicine. Veterans may qualify for free PRP therapy through the practice's Shots for Soldiers program, which offers treatment to up to two veterans per month at no charge.
