A. in American Studies from Brigham Young University in 1999, pausing his studies for a two-year LDS mission in Budapest, Hungary. D. at Tulane School of Medicine in 2003. His residency training in Family Medicine took place at the University of Nevada, Reno, where he served as chief resident during his final year.

Dr. Blake spent his early career in rural Idaho before taking an academic post as Regional Dean at Pacific Northwest University. He returned to Reno in 2012 to join the UNR faculty, serving as Vice Chair of the Department of Family and Community Medicine and as Program Director for the family medicine residency. After nearly a decade of full-time teaching, he came back to Provo to open Blake Family Medicine. His interest in headache and migraine care dates to his training at the Tulane Headache Center under Dr. Debra Elliott, and he continues to pursue headache-focused continuing education. He describes himself as a comprehensivist, managing his patients' full scope of care personally.
