Valley Ridge Direct Primary Care uses age-based monthly membership tiers. Children and young adults ages 2 to 20 pay $55 per month. Adults ages 21 to 40 pay $100 per month, ages 41 to 64 pay $125 per month, and adults 65 and older pay $150 per month. A one-time registration fee of $125 per person, or a maximum of $300 per family, covers the review of past medical records and setup of your patient chart. Families of two or more household members each receive a 10 percent discount on their memberships.

Your membership covers unlimited visits for acute and chronic conditions, same-day or next-day appointments, physicals, well woman exams, hormone therapy, and minor in-office procedures including joint injections, wart removal, skin biopsies, and laceration repair. Members also receive direct access to Dr. Adamchak by text, phone, and email, plus specialist referral coordination and home visits in select situations. The practice charges no copays and no per-visit fees for routine care.

The practice cannot currently accept patients who are enrolled in or eligible for Medicare.
