All memberships begin with a two-month trial billed at a flat $195 fee, due at the time of your first visit. After the trial, membership continues month-to-month at $45 per month. You can cancel at any time after the trial period ends.

The membership is only available to out-of-network and uninsured patients. Patients with Medicaid, Medicare, or in-network commercial insurance are not eligible. Those patients receive care billed through their insurance as normal.

Yes, state law requires a $15 fee per in-person visit, except for one annual wellness visit included in the membership. The annual flu shot is also included. Other vaccines, such as Hep B, Tdap, and Td, carry an additional charge to cover the cost of the vaccination.

No, the practice does not bill insurance for membership services, and members cannot seek reimbursement from their insurance company for monthly fees. Your insurance still applies normally to labs, specialty referrals, and ancillary services.

No, the membership is not health insurance. Urban Family Doctor encourages all patients to obtain health coverage. Uninsured patients in New York can explore options through NY State of Health or the NYC Care Program.

The membership covers preventive care, routine screenings, sick visits, chronic disease management, care coordination, STI testing, PrEP management, transgender care, routine gynecologic care, contraceptive counseling, pre-operative risk assessments, phlebotomy, and in-office procedures like ear wax removal and wound evaluation.

Let the practice know right away. Urban Family Doctor will terminate your membership and refund any membership fees on a pro-rated basis. Your visits will then bill to your insurance, and standard co-pays and deductibles will apply.

Re-enrollment is possible but not automatic. You would need to start a new two-month trial. If the practice is full, re-enrollment may not be available. The practice does not offer refunds or partial refunds regardless of usage.

Urban Family Doctor uses the Elation Passport patient portal for patient communications and records. Members also have access to the practice for general medical questions and guidance between visits.

Out-of-network and uninsured patients can sign up for the two-month trial membership online. Medicaid, Medicare, and in-network insurance patients should call the office to schedule a new patient appointment directly.
