Individual membership at Trellis Health costs $119 per month. Members can also pay on an annual basis. MaineHealth employees with qualifying Aetna plans pay just $60 per year. Some employer partners and Mending Health insurance subscribers may have their membership fully covered at no personal cost.

Your Trellis membership covers all Trellis care and services with no copays, no deductibles, and no surprise bills. You get unlimited access to a care team that includes primary care providers, physical therapists, behavioral health clinicians, pharmacists, and health coaches. The membership also includes same-day appointments, telemedicine visits, and a 24/7 phone hotline for after-hours needs.

Trellis Health does not bill insurance for its services. The membership fee covers all care directly. However, Trellis does work with employer-sponsored arrangements and Mending Health, an insurance partner that fully covers the membership for qualifying subscribers.

You can sign up directly through the Trellis Health website. Individual and family monthly memberships are available to the public. If your employer partners with Trellis or you carry a Mending Health plan, you may activate your membership through those channels instead.

Yes. Trellis Health offers same-day and next-day appointments. The practice also provides 24/7 phone support for urgent needs that arise outside of regular hours.

Yes. Trellis Health offers telemedicine visits in addition to in-person care. Members can connect with their care team by video, phone, or secure messaging.

Trellis goes well beyond a standard primary care visit. The practice offers behavioral health therapy, physical therapy, medication management, health coaching, preventive gynecology, lab tests, minor procedures including suturing and joint injections, skin biopsies, and specialist referral coordination. All services fall under a single membership fee.

Every new Trellis member starts with a Hello! Visit. During this appointment, your care team reviews your health history, past care, and personal health goals. The visit helps build a personalized care plan tailored to your needs, not just your symptoms.

Yes. Trellis partners with a number of local employers, including Luke's Lobster, Aroma Joe's, Stone Coast Fund Services, The Boulos Company, and Boulos Asset Management. Employer-sponsored memberships give employees and their families access to the full Trellis care team, often at reduced or no cost.

Trellis Health currently operates at 1945 Congress St in Portland, Maine. The practice plans to open a second location at Cook's Corner in Brunswick in 2026. Trellis also plans to expand access to Medicaid and Medicare patients in the future.
