TLJ Aesthetics MD & TLJ Concierge Primary Care

TLJ Aesthetics MD handles all aesthetic and dermatologic procedures, while TLJ Concierge Primary Care, also called CloseCare, delivers membership-based internal medicine. Aesthetic care does not require membership. Primary care services do require a CloseCare membership, and those benefits fall outside the scope of insurance.

Prospective patients apply directly through the practice. Because CloseCare limits its panel size to protect care quality, a waiting list is not uncommon. The practice encourages interested patients to connect early to check availability.

CloseCare members receive priority scheduling, same-day or next-day availability, and lengthy appointments. Members also get 24/7 access to Dr. Jacobson by phone, messaging, or appointment. The practice manages a wide range of conditions, including hypertension, diabetes, thyroid issues, perimenopause and menopause, depression, anxiety, and preventive care.

TLJ accepts insurance for medical and dermatologic care. Cosmetic procedures are out-of-pocket expenses. CloseCare primary care membership benefits fall outside insurance coverage, so the practice bills those fees directly to members.

Yes. Dr. Jacobson offers telemedicine visits for both primary care and aesthetic consultations. The practice also provides a virtual consultation tool on its website for prospective aesthetic patients.

The Beauty Secrets Rewards Program gives aesthetic patients ongoing savings across four tiers. Essential Secrets runs $120 per month, Ideal Secrets $200 per month, Perfect Secrets $300 per month, and Elite Secrets $400 per month. Each tier includes discounts on Botox, aesthetic treatments, and clinical skincare products.

The practice uses Sciton technology throughout its aesthetic menu, including BBL, BBL Hero, Halo hybrid laser, ClearSilk, SkinTyte BBL, and ClearV for vein treatment.
