Thrive Direct Primary Care

Individual adult membership at Thrive Direct Primary Care costs $85 per month. Two adults from the same family pay $150 per month. Children aged 12 to 18 pay $45 per month with an adult member or $75 per month without one. Families with two adults and children pay a maximum of $250 per month.

The membership covers all office visits, including annual physicals, chronic disease management, acute illness, urgent care, and in-office procedures such as EKGs, ear lavage, joint injections, skin tag removal, abscess drainage, and laceration repair. There are no co-pays or deductibles. Members also access labs, medications, and imaging at negotiated cash or wholesale prices.

Thrive does not bill insurance for membership or office visits. The practice contracts directly with patients. Dr. Cozart can work with any insurance plan, including Medicare, for outside labs, imaging, medications, and specialist referrals. Medicare patients sign a waiver to use these services.

Yes. Thrive accepts patients aged 12 and up regardless of insurance status. The membership covers comprehensive primary care, and the practice provides labs, medications, and imaging at transparent, at-cost prices. There are no surprise bills.

Yes. Thrive offers same or next day appointments. Because the practice keeps a smaller patient panel, Dr. Cozart has the time to see members quickly. Appointments run 30 to 60 minutes, and Dr. Cozart typically greets patients at the door.

Yes. Dr. Cozart offers virtual visits when applicable. Members can also reach him directly by phone, text, or email for questions or concerns that do not require an in-person visit.

Thrive provides labs at negotiated cash rates. 00. Medications are dispensed at wholesale cost. 05 per tablet and insulin lispro runs $35 per pen.

Yes. Thrive offers two special deals. If you pay 11 months ahead, you receive one month free. If you refer a friend who signs up, you each receive a free month of membership.

Yes. Thrive offers small business and employer plans at a flat monthly fee per employee. This gives employees access to the same direct primary care services included in individual memberships.
