Stratos DPC

Monthly fees at Stratos DPC vary by age group. Members from birth to age 25 pay $65 per month. Adults ages 26 to 64 pay $85 per month. Seniors ages 65 and older can enroll in the Amber VIP plan at $130 per month, which includes home visits. All new members also pay a one-time, non-refundable enrollment fee of $80 per individual, $150 per couple, or $230 per family.

Your monthly fee covers same-day or next-day office visits lasting 30 to 60 minutes, telehealth visits, and 24/7 physician access via text, phone, email, and video chat. The membership also includes urgent and sick care, preventive care, chronic disease management, well-woman exams, well-child visits, sports physicals, minor procedures, osteopathic manipulation therapies, IV infusion therapies, in-office ultrasound imaging, and much more. Stratos DPC does not charge copays, deductibles, or hidden fees beyond the monthly membership.

Stratos DPC does not bill insurance for DPC membership services. The practice does not participate in HMO plans. Medicare patients may enroll but must sign a waiver indicating that Stratos DPC will not bill Medicare for DPC services. The practice encourages members to carry a high-deductible or healthcare sharing plan for emergency, surgical, or specialist care.

Monthly fees start on the first of the month after enrollment. Stratos DPC accepts cash, check, and ACH bank transfers, which qualify for cash price discounts. Stratos DPC serves patients of all ages, from newborns through seniors. Pediatric patients under 18 must have an enrolled parent or guardian on file unless they are an emancipated minor. Dr. Tezaguic's geriatric fellowship training makes the practice especially well-equipped for older adult patients.

Yes. Stratos DPC members access wholesale cash pricing for most laboratory tests and radiological studies, including MRI and CT scans, at discounts of up to 90% off regular pricing. These savings apply when you pay by cash, check, or ACH transfer.

Stratos DPC provides care for uncomplicated mental health concerns. The practice does not treat complex mental health issues, chronic pain conditions, or routinely prescribe controlled medications for chronic conditions. It also does not serve as a Workers' Compensation provider.

Members who wish to cancel must provide 30 days of advance written notice via email or typed letter. Any outstanding account balances must be paid in full at the time of cancellation. The practice may cancel a membership if monthly fees remain past due by 30 days.
