Complex & Special Healthcare Needs Plan includes Standard Plan plus higher frequency visits for care management and plan oversight at $200/month for first 3 months then reassessed. Family maximum of $250/month no matter how many children enrolled. No enrollment fee. Financial assistance may be available. Non-member sick visits $120 per child per visit. Single visit consults $120 per hour.
