Testing, imaging, procedures, and visit complexity bill to insurance separately.

SilverHeart does not bill insurance for the membership fee. Patients pay the annual fee directly. Billable medical services, including visit complexity, testing, imaging, and procedures, do bill to your insurance. If Dr. Silver is out of network, the practice provides documentation to support out-of-network reimbursement.

SilverHeart schedules urgent appointments within 48 hours. When an in-person visit cannot happen that quickly, Dr. Silver offers a telemedicine visit instead. For true emergencies, he can conduct a home visit for an additional $600 fee.

SilverHeart Apex is a cardiometabolic recovery and wellness platform starting at $2,500 per year. It gives patients a dedicated care team, connected home monitoring, exercise and nutrition coaching, behavioral health support, and a 24/7 nurse line. Private Membership gives patients Dr. Silver himself: unlimited unhurried visits and his personal cell number around the clock. The two products are independent and stackable. Many members hold both.

Yes. SilverHeart Apex is open to everyone, with no Private Membership required. Referring physicians can also enroll patients directly in Apex for co-managed cardiometabolic care between office visits.

When Apex members call the nurse line at any hour, a live nurse answers. That nurse has the patient's chart available and can answer clinical questions, review home monitoring data, coordinate the care plan, and escalate to Dr. Silver's clinical team when needed. The line operates without automated menus or hold queues.

SilverHeart integrates with wearables and home monitoring devices. Blood pressure, heart rate, weight, and other cardiac data flow continuously from the patient's home to the care team. The team reviews those readings to catch trends early and adjust care plans before a visit becomes necessary.

Cardiosity is a cardiovascular education curriculum and community platform currently in development. It costs $2,000 per year as a standalone product and is included in all Membership plus Apex bundles. Dr. Silver is accepting up to 100 founding members at a one-time fee of $1,500, which provides lifetime access with no annual renewal.

Yes. SilverHeart Apex is available at $250 per month. Private Membership is $4,000 paid in full, with bundle options that split the Apex portion into monthly payments after an upfront membership payment. The practice offers several pricing combinations depending on which products patients choose.

Dr. Silver stays actively involved during hospitalizations. He communicates with the inpatient care team, can visit patients in person, and offers daily telemedicine check-ins. He remains available to family members and coordinates post-discharge follow-up directly, with priority scheduling for the first appointment after discharge.
