Membership at Schuylkill Medical Associates costs $750 per month. That fee covers home visits, all paperwork, coordination of care, monthly communication, and direct access to Dr. Pluta's cell phone and email. Two add-on fees apply: a $50 travel fee for patients located more than 15 miles from Dr. Pluta's home, and a $50 fee for gastrostomy or tracheostomy tube changes performed in the home. The practice offers month-to-month contracts with no long-term commitment required.

The practice does not accept insurance for membership fees. Patients continue to use their insurance for labs, imaging, hospitalizations, therapy, and nursing services. Members may deduct the monthly fee as a medical expense on their yearly taxes.

Yes. Dr. Pluta makes herself available to all patients around the clock. She does not see patients in the home on nights or weekends, but after-hours calls route directly to her cell phone. Dr. Pluta and her practice manager Shannon Sullivan answer all calls and emails themselves.

The practice primarily serves seniors and homebound patients, though Dr. Pluta accepts patients of any age who are homebound or find traveling to a primary care office a hardship. She sees patients in Philadelphia, Montgomery County, and Camden County.

Dr. Pluta delivers full-scope primary care in the home. Services include acute and chronic disease management, pain management, preventive care, end-of-life care, wound care, incontinence care, medication management, and coordination of care. She also changes gastrostomy tubes and tracheostomy tubes in the home.
