Individual memberships start at $59 per month for patients ages 0 to 18 and $79 per month for adults 19 and older. Small business plans run $75 per employee per month and require at least five employees and a six-month minimum. Members who prepay for a full year receive one month free. Individual and family plans carry no long-term contracts.

No. Sankoorikal Family Medicine does not bill or accept health insurance for membership fees or visits. Members may still use their insurance for services outside the practice, such as specialist referrals, imaging, emergency care, or hospitalizations. The practice does accept a small cohort of Medicare patients, but does not accept Medicare Advantage.

Members may submit dispensed medications and self-pay lab or procedure invoices from SanFam Med for FSA or HSA reimbursement for eligible expenses. Membership fees themselves are not eligible for insurance reimbursement.

SanFam Med processes membership billing on the 1st or 15th of each month via ACH autodraft or credit card. Payments go through an encrypted billing platform inside the secure patient portal.

Yes. SanFam Med offers same-day and next-business-day visits for acute concerns. Each appointment runs at least 30 minutes, giving Dr. Sankoorikal time for a thorough history, examination, and discussion of treatment options.

Yes. Dr. Sankoorikal provides telehealth visits by video, phone, or secure messaging. He also makes house calls when needed. Members choose whichever format fits their situation.

Sankoorikal Family Medicine addresses roughly 80 to 90 percent of a patient's healthcare needs in-house. This covers preventive care, chronic condition management, acute illness, mental health support, and in-office procedures. For situations requiring surgery, imaging, or specialist care, Dr. Sankoorikal coordinates referrals and recommends maintaining health insurance coverage for those events.

Members may cancel with 30 days notice. There are no long-term contracts for individual and family plans. A re-enrollment fee may apply if a member chooses to rejoin after canceling.
