Sage Direct Care charges $125 per month for the first member. A second member adds $75 per month, a third member adds $60 per month, and a full family membership runs $300 per month. A one-time enrollment fee of $75 applies to the first member, or $150 per family. No copays apply to primary care visits.

The membership covers all primary care services Dr. Dornfeld provides, with no copays and no per-visit charges. This includes physicals, acute and urgent care, chronic disease management, women's health, pediatric care, and in-office procedures and diagnostics. Members also receive breastfeeding consultations at no additional charge.

Yes. Sage Direct Care does not bill insurance for membership fees, but members can use their insurance for services outside the practice, such as specialist visits, imaging, and inpatient care. The practice negotiates at-cost lab pricing, which sometimes costs less than running labs through insurance. Sage Direct Care pairs well with high-deductible insurance plans or cost-share member plans.

Sage Direct Care offers same and next-day appointments for members. For urgent or acute concerns, members can also reach Dr. Dornfeld directly by phone after hours.

Yes. Members can visit Dr. Dornfeld in person or online. The practice makes it easy to get care without scheduling delays regardless of the format.

No. The membership includes unlimited primary care visits with no extra charges. Some minor procedures may carry a small fee to cover disposable materials, but the practice does not cap or restrict the number of visits.

Non-members can schedule a breastfeeding consultation for $195. Sage Direct Care partners with The Lactation Network (TLN), which may cover the consultation at no cost if your insurance is in-network. The practice also provides a superbill for insurance reimbursement, and consultations are HSA-eligible. Financial hardship consideration is available.

Yes. Dr. Dornfeld sees patients from 4 months of age through adulthood. The practice covers pediatric care, newborn visits, and adolescent care, all within the standard membership.

Yes. Dr. Dornfeld and Megan Mathews, RN, perform medical ear piercing using the Blomdahl system. The Blomdahl system uses 100% hypoallergenic medical grade plastic. The practice offers first and second ear lobe piercing for ages 4 months and older. Members pay $75 and non-members pay $105, which includes earrings and an aftercare kit.

New patients enroll directly through the Sage Direct Care patient portal. The one-time enrollment fee is $75 for the first member or $150 per family. Once enrolled, the monthly membership begins and members gain immediate access to all primary care services.
