All new patients begin with an initial evaluation appointment. This 60-minute visit covers a full medical assessment, establishes your clinical baseline, and determines the best path forward. If you choose to enroll in the Comprehensive Care Membership within seven days, the cost of that initial visit applies toward your first month.

Yes, all visits currently take place via secure video. Dr. Shiari serves patients located anywhere in Wisconsin. The practice has plans to add in-office visits when feasible. Telemedicine is not a substitute for emergency care; patients should contact local emergency services for urgent or life-threatening situations.

Dr. Shiari sends lab and imaging orders electronically to facilities of your choice, such as Quest or LabCorp. She sends all prescriptions electronically to the pharmacy you prefer. Your health insurance can cover those external costs even though the practice does not bill insurance for visit fees.

Yes. Present Future Health requires a 30-day written notice to cancel, submitted by email. The practice charges no early termination fees. If you give notice fewer than 30 days before your next billing cycle, the final month's dues still apply.
