Premier Medical Concierge

Premier Medical Concierge operates on a membership model outside the traditional insurance system. New patients begin with a Comprehensive First Month for $775, which includes an extended in-home evaluation, a full review of medical records, labs, imaging, and medications, a personalized treatment plan, care coordination with existing providers, and at least two telehealth follow-up visits. After that first month, members choose between a Telehealth Membership at $375 per month or an In-Home Membership at $525 per month.

The Telehealth Membership at $375 per month includes unlimited telehealth visits, direct access to Dr. Kim, secure messaging, medication management, and lab review explained personally by Dr. Kim. The In-Home Membership at $525 per month includes everything in the Telehealth plan, plus up to 10 in-home visits per year, priority same- or next-business-day scheduling, and additional visits available at $350 each.

Premier Medical Concierge does not bill insurance directly. The practice operates entirely on a membership basis. However, patients may use HSA or FSA funds to cover membership fees. Out-of-network reimbursement through a patient's insurance plan may also be available, and some supplemental insurance policies cover concierge membership fees.

Dr. Kim makes in-home visits throughout Sacramento Valley, including Sacramento, Elk Grove, Roseville, Rocklin, Granite Bay, Folsom, and El Dorado Hills. He also serves the Central Valley, including Stockton, Modesto, Tracy, Lodi, and San Joaquin County. Additionally, he covers all nine San Francisco Bay Area counties.

Yes. In-Home Membership members receive priority same- and next-business-day scheduling. Dr. Kim brings care directly to the patient's home, which removes many of the logistical barriers that delay access to specialist care.

Yes. Both membership tiers include telehealth access. The Telehealth Membership includes unlimited virtual visits with Dr. Kim. In-Home Membership members also have full telehealth access in addition to their in-home visits. Patients can reach Dr. Kim through secure messaging between appointments.

New patients begin with a free phone consultation to determine whether Premier Medical Concierge is the right fit. If it is, the Comprehensive First Month at $775 serves as the onboarding period. It includes a full in-home evaluation, personalized treatment plan, written flare plan, and coordination with existing providers.

Premier Medical Concierge focuses exclusively on rheumatology. Dr. Kim treats rheumatoid arthritis, psoriatic arthritis, fibromyalgia, systemic lupus erythematosus, myositis, vasculitis, ankylosing spondylitis, Sjogren's syndrome, scleroderma, gout, and osteoarthritis. He also treats rarer autoimmune presentations including antisynthetase syndrome and multiple forms of vasculitis.

Dr. Kim built Premier Medical Concierge around the belief that patients with complex autoimmune and chronic pain conditions deserve care that meets them where they are. Many rheumatology patients find travel to clinic visits difficult or exhausting. Bringing care into the home allows Dr. Kim to deliver thorough, focused attention without adding physical burden to the patient's day.

Yes. Care coordination with existing healthcare providers is a core part of the Premier Medical Concierge model. Dr. Kim communicates directly with each patient's primary care physician and any other specialists involved in their care. This coordination begins during the Comprehensive First Month and continues throughout the membership.
