Monthly membership fees at Polaris Medical range by age: $30 per month for children under 18, $50 for young adults ages 18-24, $70 for adults ages 25-44, $85 for adults ages 45-64, and $100 for adults 65 and older. An enrollment fee of $100 per individual or $150 per family applies when joining. The membership covers annual physicals, all office visits, same-day sick appointments, chronic disease management, minor procedures, and after-hours physician access.

Polaris Medical does not bill any insurance plans. Members pay a flat monthly fee that covers nearly all primary care services. If you carry insurance, you can still use it for labs, imaging, and specialist visits outside the practice.

Yes. Polaris Medical requires a one-year minimum membership commitment. That continuity allows the physicians to build a thorough understanding of your health history, goals, and family context over time.

Polaris Medical treats whole families, from newborns through older adults. Children under 18 must have at least one parent or guardian enrolled as a member.

Polaris Medical physicians do not prescribe narcotic or benzodiazepine medications for long-term use. They may prescribe these medications at their discretion for acute illness or injury. If a condition becomes chronic and requires ongoing controlled substances, they refer patients to a specialist better equipped for that long-term management.
