Palisade DPC

Individual adult membership (ages 18-64) costs $100 per month. The enrollment fee equals the first month's payment, so adults pay $100 to enroll. Children under 18 pay $50 per month. A family plan covering two adults and up to four children costs $300 per month. College students pay $75 per month. Patients 65 and older pay $125 per month, though Dr. Allen currently does not accept Medicare or Medicare-eligible patients.

Membership covers the full range of primary care visits, including annual wellness exams, well child checks, sports and preoperative physicals, chronic disease management, urgent care visits, mental health care, and women's health services. It also includes a broad set of in-office procedures: laceration repair, joint injections, cryotherapy, dry needling, dermoscopy, toenail removal, skin biopsies and excisions, Nexplanon and hormonal IUD insertions, and point-of-care ultrasound. Labs and generic medications are available at wholesale pricing.

Palisade DPC does not bill health insurance. The practice keeps pricing simple and transparent through the monthly membership fee. Members can still use their existing insurance for labs, imaging, hospitalizations, and specialist visits. HSA and FSA funds are accepted for the membership fee.

Members can call, text, or email Dr. Allen directly. Palisade DPC offers same-day and next-day appointments. The practice operates Monday through Friday by appointment. Prospective patients can also schedule a free introductory meeting with Dr. Allen to explore whether DPC is the right fit.

Yes. Dr. Allen offers home visits for an additional $40 per visit. This fee sits outside the standard monthly membership and covers visits conducted at the patient's home.

Not yet. Dr. Allen currently cannot accept patients who have Medicare or are Medicare-eligible. He plans to opt out of Medicare billing once the practice reaches a sustainable size, at which point he will open enrollment to patients over 65. com.

Palisade DPC is temporarily operating out of the Lakewalk Surgery Center at 1420 London Road, Suite 100, Duluth, MN 55805. The original clinic at 520 East 4th Street is undergoing renovation following smoke damage. The practice is open Monday through Friday by appointment only.
