Membership Details

Fees billed annually. Primary member: $2,400 per year. Each secondary member: $2,000 per year. Fees can be set up on an autopayment plan. Monthly payment option available starting Jan. 1, 2026 via MyChart. Insurance billed separately for office visits, imaging, and labs. Co-payments and deductibles still required. May be eligible for reimbursement through some HSAs/FSAs.
