Oregon Concierge Medicine offers three membership tiers. Virtual Access starts at $179 per month when billed annually ($199 monthly) and includes 24/7 access by phone, text, email, or video, with in-home visits available at $239 each. Full Access starts at $329 per month annually ($349 monthly) and includes unlimited 24/7 access with in-home and office visits included. Nursing Home and Memory Care membership starts at $479 per month annually ($499 monthly). A 3% processing fee applies to credit card payments, and monthly plans require a credit card.

Oregon Concierge Medicine does not accept insurance for membership fees. The membership covers primary care services outlined in your retainer agreement. The practice recommends maintaining separate health insurance for services outside that scope, such as hospitalizations, specialist visits, and medications.

Members receive Dr. Jaczko's direct cell phone number and email address. You can contact him by phone, text, email, or video call 24 hours a day, seven days a week. He responds without gatekeepers or intermediaries.

Yes. Oregon Concierge Medicine offers same and next-day appointments. Dr. Jaczko prioritizes timely access for his members, whether for an acute issue or a scheduled visit.

Dr. Jaczko sets aside ample time for the first visit to review your full medical history. You discuss your current health issues, concerns, values, and goals at a relaxed pace. For Full Access and Nursing Home members, the initial visit includes a physical exam, lab orders, and other primary care assessments. Dr. Jaczko then works with you to build a personalized health plan.

Dr. Jaczko performs a broad range of in-person services, including joint injections, skin biopsies, wart and toenail removal, laceration repair, ear wax removal, and strep, COVID, and urine screenings.
