Individual membership at Oak Hills Medical starts at $199 per month. Family plans are available: two members cost $349 per month, three members cost $449, and four or more cost $499. A one-time enrollment fee of $99 applies, though Oak Hills Medical waives that fee for patients who choose the annual billing option.

The membership covers all care that Dr. Fowls delivers directly. This includes same and next-day appointments, video visits, texts, calls, up to six home visits per year, in-home procedures, select in-home lab tests, chronic disease management, preventive care, gynecological care, culinary medicine guidance, and immunization recommendations. Specialist care, ER visits, imaging, and reference lab tests fall outside the membership.

Oak Hills Medical does not accept insurance. The practice operates on a private-pay membership model billed monthly or annually. Members may choose to carry a separate insurance policy for specialist visits, hospitalizations, or other services outside the practice's scope.

Yes. Oak Hills Medical guarantees same or next-day access for members. Dr. Fowls also stays reachable by phone, text, email, and video between scheduled visits. After-hours access for urgent medical concerns is also available.

Each membership includes up to six home visits per year. Dr. Fowls travels to the patient's home for physical exams, sports and school physicals, acute care, procedures, and in-home lab tests. Multiple family members under the same membership can be seen during a single visit. Home visits are included within a 10-mile radius of Oakland, NJ. Additional fees apply for greater distances.

Members can also book personalized in-home cooking instruction sessions with Dr. Fowls for an additional $99 each.

Yes. Oak Hills Medical works with small businesses to create customized healthcare solutions for employees. The practice also conducts annual medical evaluations for fire and rescue departments. These evaluations include physical exams, hearing and vision screenings, pulmonary function testing, EKGs, and OSHA Respirator Medical Evaluation Questionnaire reviews.

New patients start with a complimentary virtual meet and greet with Dr. Fowls. That conversation covers your health goals and helps determine whether the practice is a good fit. After enrolling, Dr. Fowls schedules an initial home visit for a comprehensive health assessment.
