Dr. Nelly offers several membership tiers. Telehealth Membership starts at $90 per month for up to two visits. Signature Care, which includes in-office and telehealth visits, runs $140 per month for adults 26 and older and $90 per month for ages 5 to 25. Concierge Care, which adds home visits, is $450 per month for an individual or $1,400 per month for a household. A one-time enrollment fee of $99 applies to new patients.

Yes. Dr. Nelly accepts HSA and FSA funds toward membership fees. The practice does not bill insurance for visits, but provides a superbill on request for patients who want to seek possible out-of-network reimbursement on their own.

New patients can start four ways: book a single telehealth visit without a membership, sign up for a Telehealth Membership, enroll in Signature Care through the patient portal, or request a complimentary meet and greet for Concierge Care. Dr. Singh personally evaluates fit for Concierge Care and confirms home visit availability in your area.

Yes. The practice offers same-day or next-day appointments for both in-office and telehealth visits. Appointments run 30 to 60 minutes. Concierge Care members also have direct after-hours access to Dr. Singh, including weekends.

The practice does not currently accept Medicare patients. Dr. Nelly maintains a waitlist for Medicare patients and will reach out as availability opens.

Dr. Singh makes home visits through the Concierge Care tier in select Bergen and Sussex County communities. These include Sparta, Wyckoff, Mahwah, Franklin Lakes, Saddle River, Upper Saddle River, Glen Rock, Allendale, Waldwick, Ridgewood, and Ramsey, among others. She serves communities roughly within an hour of Newfoundland, NJ. Patients can request a meet and greet to confirm their location falls within the service area.

Yes. Dr. Singh offers telehealth to any patient physically located in New Jersey at the time of the visit. This includes statewide access through both the Telehealth Membership and the pay-per-visit option. Controlled substances require an in-person visit per New Jersey law.

All memberships at Dr. Nelly run month-to-month with no long-term contracts. Patients can cancel at any time. Re-enrollment after cancellation carries a $299 fee.

Most appointments run 45 minutes, with a range of 30 to 60 minutes depending on the visit type. OMT initial evaluations run 60 to 90 minutes, with follow-ups at 30 to 45 minutes. Every visit connects patients with Dr. Singh directly. The practice keeps its panel intentionally limited so each appointment receives adequate time.

Yes. Osteopathic Manipulative Treatment visits do not require a membership. An initial OMT evaluation is $250, and follow-up visits are $160. Patients can book these as standalone appointments without enrolling in any care tier.
