Yes. Premier Membership includes three at-home visits per year, coordinated by your dedicated Premier physician and delivered based on your needs. If you need additional in-home visits beyond the three included, the practice offers them at $1,200 per visit from your Premier physician.

Yes. The practice offers same-day and next-day scheduling, with appointments that start on time. The practice operates seven days a week with extended primary and immediate care hours, giving members reliable access when they need it.

Yes. Members can schedule secure video visits through the MyChart member portal. The portal also allows messaging with providers, prescription refill requests, and access to health records. The practice offers virtual nutrition counseling as well.

The practice brings together an integrated multispecialty team that includes primary care, cardiology, dermatology, gynecology, physical therapy, behavioral health, nutrition counseling, health coaching, and medical acupuncture. Health Navigators coordinate specialist referrals across the Mount Sinai Health System and beyond when additional care is needed.

It includes a comprehensive medical history review, lab analysis, musculoskeletal review, mental health session, nutritional assessment, vision and hearing screenings, dermatology consultation, health coaching, cancer screenings, advanced imaging, 3D body composition scanning, and results counseling. It forms part of the Premier Membership and gives members a detailed picture of their current health.

The flagship practice is at 55 Hudson Yards in Manhattan. A second concierge location at 110 East 60th Street on the Upper East Side serves Premier members. Both locations give members access to the full multispecialty team and on-site services including X-ray and lab testing.

The practice does not accept insurance for membership fees. However, Health Navigators assist members with insurance coordination and billing for services, and many clinical services may still bill through insurance. Contact the practice to clarify coverage for specific services.

Members use the MyChart portal to schedule appointments, message their care team, request prescription refills, view lab results, and join video visits. The practice also sends members a monthly newsletter with health tips from its panel of medical experts.
