Moonstone Direct Primary Care

Monthly membership fees at Moonstone Direct Primary Care start at $99 per month for children ages 4 to 17 with an accompanying adult member. Adults ages 18 to 39 pay $129 per month, adults ages 40 to 64 pay $149 per month, and adults 65 and older pay $199 per month. A one-time non-refundable enrollment fee of $200 per person applies to new patients. Students receive a discounted rate of $109 per month with proof of current enrollment, and families of four or more receive a 10% discount on monthly fees.

The monthly fee covers all primary care services with no additional copays. This includes preventive care like annual physicals, well child checks, and well woman exams, as well as chronic disease management, acute care, mental health support, and care navigation. Labs, imaging, immunizations, and medications fall outside the membership but members access discounted cash-pay prices for labs and imaging.

Moonstone Direct Primary Care does not bill insurance for primary care services. Members pay the flat monthly fee directly. Patients can continue using their insurance for specialist visits, imaging, labs, medications, and hospital or emergency care. Moonstone DPC recommends carrying some form of health insurance or health share for larger expenses.

Yes. Same-day and next-day appointments are often available. Members also reach Dr. Dhami by phone or text for guidance between visits, making urgent concerns easier to address quickly.

Dr. Dhami sees patients through in-office visits at the Santa Clara clinic, home visits across the South Bay, and secure telehealth. Phone and text access is also available. Members work with Dr. Dhami to find the visit format that fits their needs.

The office is located at 2391 The Alameda, Suite 110, Santa Clara, CA 95050. Dr. Dhami sees patients there on Tuesdays and Fridays by appointment. Home visits are available across Milpitas, Santa Clara, San Jose, Sunnyvale, Mountain View, Los Altos, Cupertino, Campbell, Saratoga, and Palo Alto.

Yes. Dr. Dhami offers secure virtual visits for routine primary care and ongoing care management. Telehealth works well for follow-ups, medication reviews, and situations where an in-person visit is not necessary.

Medicare patients currently join a waitlist at Moonstone Direct Primary Care. Interested Medicare patients can contact the practice directly to add their name.

Prospective patients can schedule a complimentary meet-and-greet with Dr. Dhami to discuss their health needs and confirm the practice is a good fit. From there, enrollment involves a one-time enrollment fee and the start of the monthly membership.

Yes. Dr. Dhami leads a monthly Walk with a Doc community walk at Gill Memorial Park in Milpitas every second Saturday of the month. Each walk features a health topic discussion. No membership is required to attend.
