John Chiu, M.D.

D. covers comprehensive primary and preventive care for a flat monthly fee. Members receive unlimited sick visits, annual physicals, chronic illness management, minor procedures, EKGs, audiograms, joint injections, and blood draws. Phone and email access to Dr. Chiu is also included. The program does not cover hospitalizations, specialist care, or outside lab fees.

D. are $50 for patients aged 0-18, $60 for adults aged 18-64, and $75 for patients 65 and older. The practice charges no enrollment fee. Family discounts apply: the oldest parent pays full price, and spouses and children receive a 10% discount.

Yes. Dr. Chiu participates as a preferred provider with most major insurance companies. Patients who prefer to use insurance for standard visits may do so. The DPC program operates separately and works well alongside a high-deductible insurance plan.

Yes. The practice accommodates urgent appointments for acute problems that need evaluation within 24 hours. Walk-in patients also receive consideration for same-day availability when the schedule allows. Patients should call the office first to confirm availability.

The DPC program requires a one-year service contract. Patients who cancel during the first year pay an early cancellation fee equal to the remaining months on the contract. After the first year, membership converts to month-to-month with no cancellation fees.

Dr. Chiu performs blood draws at his Los Gatos office. Patients who prefer an outside facility may use Quest Diagnostics, LabCorp, or El Camino Hospital.

Yes. Dr. Chiu offers travel vaccines for patients preparing for international travel, in addition to all routine vaccines for children, teens, adults, and seniors.
