The Foundations plan starts at $4,500 per year. The Optimization and Executive tiers carry custom pricing based on the patient's goals and selected services. Each tier builds on the previous one with additional diagnostics and monitoring capabilities.

The Foundations plan includes five care visits per year, two of which are directly with Dr. Nguyen. Members also receive an 80-plus biomarker lab panel, strength and endurance testing, VO2 max testing, body composition analysis, biological age assessment via PhenoAge, and an annual progress review.

The Optimization tier adds a DEXA scan, mid-year lab draws at months three and six, and 24/7 wearable tracking covering HRV, sleep, and recovery. The Executive tier includes Galleri multi-cancer early detection testing and epigenetic age testing on top of Optimization benefits.

The Lifetime Body Score is a proprietary composite number Dr. Nguyen developed to summarize a patient's health across six domains: Metabolic Health, Cardiovascular Performance, Body Composition, Physical Performance, Recovery and Resilience, and Biological Age. The score drives the patient's personalized 12-month improvement plan.

Lifetime Performance Medicine operates on a direct membership model. Members pay an annual fee for their tier. The practice does not rely on insurance networks to determine the scope or pace of care.

The practice operates Monday through Friday from 9:00 AM to 5:00 PM at its Los Gatos location. Patients should contact the practice directly to discuss visit formats available at each membership tier.
