Mid-Valley Direct Primary Care

Individual adult membership costs $90 per month. The practice offers several tiers, including $50 per month for adults ages 19 to 29, $100 per month for adults 65 and older, and up to $200 per month for a two-parent family with unlimited children through age 24. Members who pay annually receive a 5% discount.

The membership covers unlimited office visits and phone or text visits at no additional charge. It also includes a broad range of in-office procedures such as laceration repair, joint injections, EKG, spirometry, blood draw, and many others. Lab tests, imaging, immunizations, and durable medical equipment cost extra.

The practice does not accept insurance or work with any third-party payers. Members with existing insurance can keep their plans, but Mid-Valley DPC will not bill them. The practice works well alongside high-deductible insurance plans, HSAs, and cost-sharing co-ops.

Mid-Valley Direct Primary Care offers same-day or next-day appointments for most needs. Members can also reach Dr. Bain by phone or text for appropriate issues, and the practice provides 24/7 on-call physician coverage.

Members pay directly for lab tests and imaging at negotiated rates. Common lab tests run $5 to $15 each. Most X-rays cost $50 to $60, most CT scans run $200 to $400, and most MRIs run $300 to $500. The practice coordinates with Salem Radiology Consultants and Advanced Imaging for these services.

Most minor procedures carry no additional charge. These include laceration repair, toenail removal, injections, wound care, wart destruction, IUD insertion and removal, and several others. A visit requiring a culture costs $30. A biopsy costs $50. All other procedure fees apply only to lab tests, imaging, immunizations, and DME obtained outside the office.

Yes. Dr. Bain offers phone and text visits for appropriate situations at no additional charge. These visits count toward your membership and require no additional fee.

Yes. The practice pairs well with high-deductible plans, HSAs, and cost-sharing co-ops. Those plans cover major unexpected expenses, while the DPC membership covers routine primary care at predictable monthly rates.
