MALI offers multiple membership tiers. The Core Integrative Membership starts at $150 per month and covers primary care, hormone and metabolic health, sexual health, chronic disease management, and preventive care. The Complete Care Integrative Membership is $245 per month and adds a monthly personal training session, unlimited body composition scans, and exclusive pricing on compounded GLP-1 medications. A focused Metabolism and Menopause membership is also available at $118 per month for patients who need targeted hormone and metabolic support without full primary care. All memberships carry a six-month minimum commitment.

Members pay one flat monthly fee with no co-pays and no insurance billing for primary care services. The fee covers all primary care visits, same-day or next-day sick visits, telehealth appointments, nutritional guidance through the patient portal, and direct access to Dr. Blecher. Your existing insurance still covers labs, imaging, specialist visits, prescriptions, and emergency care. MALI has also negotiated discounted cash-pay rates with local labs and imaging centers.

Yes, starting January 1, 2026, federal law allows HSA funds to cover qualifying DPC membership fees. MALI accepts HSA payments, with caps set at $150 per month for individuals and $300 per month for families. Eligibility depends on your specific HSA plan provider, so check with your plan before enrolling.

Start by completing the Become a Patient form on the MALI website. Then schedule a complimentary virtual meet-and-greet with Dr. Blecher by phone or video. If you decide to move forward, you complete enrollment paperwork electronically. Once Dr. Blecher reviews your paperwork, she sends a confirmation email and you schedule your first visit to establish care.

MALI offers same-day and next-day appointments for members. Initial visits run a full 60 minutes. The practice caps its panel at 150 patients, which keeps availability consistent for everyone in the practice.

MALI does not bill insurance for primary care services. Members pay a flat monthly fee directly to the practice. Your insurance remains active and covers labs, imaging, specialist referrals, prescriptions, and emergency visits. Some members also choose to use MALI's negotiated cash-pay rates for labs and imaging rather than running them through insurance.

Yes. Dr. Blecher offers telemedicine appointments for members who prefer remote visits. Some follow-up visits can also take place outdoors on a walk or in the on-site gym, depending on the nature of the visit and patient preference.

MALI does not accept Medicaid patients at this time. Medicare-eligible patients ages 65 and older should contact the practice directly to discuss their individual situation. Dr. Blecher welcomes the conversation and works with each prospective patient to find a path forward where possible.

All MALI memberships require a six-month minimum commitment. After that period, members who wish to cancel must provide 30 days written notice. This structure allows Dr. Blecher to maintain meaningful, ongoing care relationships and plan appropriately for her panel.
