MD² Chicago Gold Coast limits its panel to 50 families. That size gives each member direct, unhurried access to both Dr. Tellez and Dr. Evan Lu.

The membership covers every primary care service the practice provides. This includes an extensive annual physical, unlimited visits, house and workplace calls, x-rays, routine labs, blood panels, cardiovascular screening, and cancer evaluations. Members receive no bill when they leave the office.

Yes. Members receive direct access to both physicians' private cell phones, available any time of day or night. You never need to go through a front desk or answering service to reach your doctor or their partner.

Yes. Your MD² physician will guide you to appropriate specialists and often accompanies you to those appointments. This ensures complete continuity of care throughout any specialist or surgical referral.

Yes. Your physician remains your primary medical contact regardless of where you travel. MD² Chicago Gold Coast also coordinates care with partner MD² offices in other cities if you need attention while away. Members also enroll in a worldwide medical evacuation service.

Your MD² physician visits you in the hospital and manages your care directly. The practice does not hand patients off to hospitalists. Your doctor acts as your advocate and coordinates with any specialists involved in your care.

MD² Chicago Gold Coast does not lock members into a binding contract. The agreement the practice uses reflects its commitment to you, not the other way around. You can cancel your membership at any time without financial penalty, and your doctor will help you transition to another physician if you choose to leave.
