Manifest Health Concierge Medicine

Manifest Health Concierge Medicine charges $100 per month or $3,000 annually. The membership covers all office visits with no copays, virtual consultations, direct follow-up communications, test result reviews, treatment plan discussions, specialist coordination, and 24/7 direct access to Dr. Koza. Laboratory work, imaging, and specialist care fall outside the membership and bill separately.

Manifest Health does not file insurance claims. Patients may submit claims to their insurer directly for potential out-of-network reimbursement. The practice also accepts HSA funds for membership payments. For Medicare patients, Dr. Koza participates as a non-participating provider but still coordinates referrals to specialists, hospitals, and imaging facilities.

Yes. Members contact Dr. Koza directly 24 hours a day, 7 days a week by secure messaging, phone, or email. This direct access is included in every membership. The practice also offers extended Monday evening hours from 10am to 7pm and early Thursday morning hours from 7am to 3pm.

Appointments run 30 to 60 minutes, with a typical visit lasting around 45 minutes. Dr. Koza also offers same-day and next-day appointments. This allows enough time to address concerns thoroughly and build a genuine understanding of each patient's health.

Manifest Health offers comprehensive family medicine including annual physicals, preventive care, chronic disease management, mental health care, obesity management, and women's health services. Dr. Koza also performs a wide range of in-office procedures including joint injections, IUD placement and removal, Nexplanon insertion and removal, cryotherapy, biopsies, lipoma and cyst removal, and toenail removal. Point-of-care testing for blood sugar, HbA1C, pregnancy, urinalysis, flu, COVID, RSV, and strep is also available in the office.

Yes. Women's health is a central focus of the practice. Dr. Koza holds the Menopause Society Certified Provider (MSCP) designation and manages menopause, perimenopause, hormone changes, contraceptive care, and conditions specific to women at every life stage. She built the clinical program to go beyond what a standard primary care practice typically offers in women's health education and services.

Yes. Dr. Koza offers virtual consultations as part of every membership. Telehealth visits work well for follow-ups, prescription management, test result reviews, and many non-urgent concerns. Members schedule these through the athenahealth patient portal.

Yes. Manifest Health offers a special student membership tier for CU Boulder students. The practice serves Lafayette and the broader Boulder County community, and Dr. Koza designed the practice to be accessible to patients across a range of life stages and schedules.
