Members 65 and older pay $110 per month. A one-time enrollment fee of $100 for individuals or $150 for families applies at sign-up. Primary care visits carry no copay.

LIGHT Family Wellness operates on a direct-pay membership model and does not bill insurance for primary care services. However, the practice partners with Atlas Direct supplemental insurance, which members can add to complement their DPC membership. Yes. LIGHT Family Wellness offers three GLP-1 options. GLP-1 Medical Oversight starts with a $149 metabolic consultation followed by $139 per month with a four-month minimum. Compounded GLP-1 begins with a $99 consultation, with medication running $159 to $439 per month depending on the formulation. Dr. Maxwell provides physician-led oversight throughout the program.
