Jeffrey H. Graf, MD

At Jeffrey H. Graf, MD, a board-certified internist and cardiologist brings more than 30 years of solo private practice to the Upper East Side of Manhattan. Dr. Graf launched his concierge program on June 1, 2012, building on a model he had already offered patients for years before that. He keeps his panel at 150 members, giving each one the time and attention their care requires.

Dr. Graf holds dual board certification in internal medicine and cardiology. His clinical focus spans cardiovascular disease prevention, high blood pressure, high cholesterol, weight management, chest pain, and palpitations. He performs diagnostic testing in-office, including echocardiographic examination, cardiac stress testing, and EKG. Through his collaboration with Castle Connolly Private Health Partners to form Graf CCPHP, LLC, members also access the SENS Solution Wellness Program, a structured approach to sleep, exercise, nutrition, and stress management. Some patients have trusted Dr. Graf with their care across three generations of their families.
