Hydrangea Family Medicine

Adults 19 and older pay $135 per month. Children ages 0 to 18 pay $85 per month. Families of up to two adults and two children pay a maximum of $325 per month. A one-time enrollment fee equal to one month's membership applies at contract signing, and the practice requires a minimum enrollment of three months.

The monthly fee covers all primary care services Dr. Garofalo provides in the office. That includes in-person and virtual visits, phone calls, texting, and portal messages. It also covers in-office procedures like skin tag removal, wart removal, ear wax lavage, wound care, and EKGs. There are no copays, deductibles, or hidden fees.

No. Hydrangea Family Medicine does not accept insurance, including Medicare. Dr. Garofalo has opted out of Medicare. Members pay the flat monthly fee directly for all care provided in the office. You can still use your insurance or Medicare benefits for services outside the practice, such as specialist visits, imaging, or medications.

Yes. Dr. Garofalo has opted out of Medicare, so she does not bill Medicare for any in-office services. Medicare patients pay the same flat monthly membership fee as other members. You retain your Medicare benefits for outside services such as specialist visits, radiology, and prescriptions, all of which Dr. Garofalo can order for you.

Hydrangea Family Medicine offers same-day or next-day access for sick visits. This lets members skip urgent care centers or emergency rooms for issues Dr. Garofalo can handle in the office. You can also reach her by phone, text, or secure portal message for guidance before deciding whether you need to come in.

Yes. Dr. Garofalo conducts virtual visits as part of the standard membership. Telemedicine visits carry no additional fee. When a virtual visit can resolve your concern, you save the trip to the office entirely.

Yes. Dr. Garofalo makes home visits when indicated and feasible. Home visits carry an additional charge of $50 per visit, separate from the monthly membership fee.

Yes. Membership at Hydrangea Family Medicine covers primary care services only. You still need health insurance for hospitalization, surgery, specialist visits, imaging, and medications obtained outside the practice. Massachusetts residents are required to carry health insurance.

HMO plans typically require an in-network primary care physician to authorize specialist referrals. Dr. Garofalo does not participate in insurance networks, so she will not serve as an in-network PCP. Prospective patients with HMO plans should contact the practice to discuss how membership would work alongside their specific plan.
