Hometown Family Health charges a flat monthly fee based on age. Members ages 0-19 pay $65 per month. Members ages 20-49 pay $85 per month. Members ages 50-65 pay $100 per month. A one-time enrollment fee of $50 per person applies, with a family maximum of $150. Employer rates are also available.

The membership covers all visits with no copays or per-visit fees. Members also receive direct communication with their doctor via text, phone, or email. The practice performs minor in-office procedures and osteopathic manipulative medicine for acute conditions. Members receive wholesale pricing on labs, imaging, and medications. Lifestyle medicine resources, including certified personal trainer and nutrition coaching services, are also part of the membership.

Hometown Family Health does not accept insurance for membership fees or visits. The practice declines insurance reimbursements so it can pass wholesale pricing directly to members on labs, imaging, and medications. Members may still carry insurance for hospitalizations or specialist visits outside the practice.

Yes. The practice offers same and next-day appointments. Keeping the panel capped between 500 and 600 patients allows the practice to maintain this level of access for members.

Yes. For urgent situations after hours, Dr. Cardon is available to members. The practice asks that patients call rather than text or email for urgent after-hours needs.

Yes. Hometown Family Health offers telehealth visits for members. The practice also provides standalone virtual appointments for menopause and perimenopause support, which are open to non-members.

Hometown Family Health sees patients from newborns through age 65. The practice serves children, adolescents, and adults, including care for pediatrics, women's health, and chronic disease management. The practice does not see Colorado Medicaid or Medicare patients, or individuals 65 and older.

Hometown Family Health offers dedicated menopause and perimenopause support through virtual appointments. This service is open to both members and non-members. The first visit costs $250, and follow-up visits cost $150. The practice provides evidence-based, personalized support for symptoms like hot flashes, brain fog, sleep disruption, and mood changes.

The Lifestyle Medicine program addresses root causes of chronic conditions through nutrition, exercise, sleep hygiene, and stress management. The program includes extensive lab work, personalized nutrition and exercise plans, and sleep and stress resources. Allison Cardon, a NASM-certified personal trainer and nutrition coach, contributes to member lifestyle plans.
