At Home Based Medicine, a team of four physicians brings primary and palliative care directly to patients who cannot easily travel to a clinic. The practice serves homebound adults across the San Francisco Bay Area, reaching patients in private homes, residential care facilities, assisted living communities, and senior living homes. Dr. Gurpreet Padam founded the program and has spent over 24 years focused on adult medicine and care for the homebound. The practice operates in affiliation with UCSF Health and draws on over 40 years of cumulative physician experience. The clinical team handles a wide range of services during home visits, including chronic care management, cognitive evaluations, palliative care, goals of care conversations, medication reconciliation, and advance care planning. Through local radiology partners, the practice coordinates in-home blood work, EKG, X-ray, and ultrasound. Medicare covers Remote Patient Monitoring for qualifying patients, and a Chronic Care Management program supports patients managing two or more chronic conditions.
