Helios Health AZ - Direct Primary Care & Wellness

Individual membership costs $85 per month or $969 per year when paid in advance. Couple memberships run $135 per month and family memberships for up to four members cost $150 per month. Each additional child beyond four adds $10 per month. Paying annually saves 5% off the monthly rate.

Membership covers direct access to Dr. Wesley by text, email, phone, and telehealth. It also includes same-day or next-business-day appointments, extended in-person visits, minor procedures, and transparent pricing on labs and imaging. Members also receive a comprehensive yearly evaluation with InBody testing, VO2Max testing, EKG, spirometry, point-of-care ultrasound, vision and hearing screenings, and basic labs at discounted pricing.

Yes. Members contact Dr. Wesley directly by text, email, or phone. Telehealth visits are also available. Many questions and minor concerns resolve without requiring an in-person appointment, saving members time and effort.

The practice does not bill insurance for membership services. Dr. Wesley works directly for his patients, not insurance companies. Members can still use insurance coverage for specialists, imaging, and other services outside the practice. Most DPC members choose a high-deductible plan to cover major health events.

Medicare patients are welcome to join the practice. However, Helios Health AZ has opted out of Medicare, which means neither the practice nor the patient can bill Medicare for services provided here. Medicare patients retain their benefits for outside services such as labs, imaging, and specialist visits.

Call the office at 928-660-6960 to discuss your healthcare needs and request official membership forms. The practice guides new patients through the enrollment process from there. Existing members access their records and communications through the Elation Passport patient portal.

Yes. Dr. Wesley conducts telehealth visits as part of the standard membership. Telehealth is a convenient option for questions, follow-ups, and issues that do not require a physical exam. Members do not pay extra for telehealth visits.

Yes. Helios Health AZ offers hormone therapy through oral, topical, and EvexiPEL pellet delivery options. The one-time hormone consultation fee is $100, but the practice waives it for members. Pellet therapy costs $350 for women and $750 for men.

The practice does not currently offer immunizations. Patients can access immunizations at local health departments and pharmacies. Dr. Wesley focuses on the comprehensive primary and preventive care services that fall within the DPC membership model.

Members cancel with 30 days of written notice. If a former member decides to re-enroll, the practice requires written approval and a $225 re-enrollment fee before reinstating membership.
