Healthy Self DPC

The standard DPC membership runs $100 per month for a single member, with a six-month minimum commitment. Healthy Self DPC charges an $80 enrollment fee per person, reduced to $50 per person for families of three or more. Members who pay annually in full receive a 10% discount. Special rates apply for veterans, teachers, first responders, and college students.

Members receive full-scope family medicine care including acute and chronic disease management, annual wellness visits, well child and well woman exams, same-day urgent appointments, 60-minute visit times, telehealth, house calls, and wholesale lab pricing. The membership covers a wide range of in-office procedures from laceration repair and joint injections to EKGs and Pap smears.

The Functional Approach Membership costs $150 per month and runs separately from the standard DPC membership. It covers hormone replacement therapy, peptide therapy, GLP-1 management, IV therapy, and aesthetic services. Patients interested in this level of care enroll in both memberships.

Healthy Self DPC does not bill insurance for membership services. Members pay the practice directly each month. The practice recommends pairing the membership with a high-deductible health plan or health sharing plan to cover hospitalizations and specialist care outside the practice.

Yes. Healthy Self DPC offers employer plans at $80 per month per employee. Employers enrolling more than 10 employees pay a $50 enrollment fee per employee. Healthy Self DPC sees patients from one day old through age 99. Members who reach 100 years of age receive their membership free of charge. The practice handles pediatric care, adolescent care, adult medicine, and geriatric care all under one roof.

Osteopathic Manipulation Medicine occupies half of Dr. Eller's clinical practice. She uses techniques including myofascial release, cranial sacral therapy, visceral manipulation, and HVLA, among others. OMT serves patients managing chronic pain, musculoskeletal conditions, structural issues, and a range of systemic concerns.

Yes. Members access wholesale lab pricing, with common tests available at a fraction of retail cost. The practice also offers specialized testing including GI-MAP gut microbiome analysis, environmental and food allergy panels, methylation genetic variant testing, the Galleri multi-cancer detection screen, CIMT carotid artery scanning, and expanded cardiac risk scoring.

Prospective patients can schedule a complimentary meet-and-greet with Dr. Eller before enrolling. The practice accepts members on a rolling basis. New members select their membership tier, pay the enrollment fee, and establish their first appointment with Dr. Eller.
