Members pay an annual membership fee to Graf CCPHP, LLC for these enhancements. Separately, Dr. Graf's practice bills insurance or Medicare for professional medical services. Yes, the practice participates with several health insurance plans, including Medicare. Insurance covers professional services, subject to applicable deductibles, copayments, and coinsurance. The annual membership fee is not a medical charge and members should not submit it to a health insurer for reimbursement.

Members may choose to submit the membership fee to a flexible spending account, health reimbursement account, or employer medical savings account. Graf CCPHP makes no representation that the fee qualifies for reimbursement from those accounts. Members should confirm eligibility with their plan administrator.

Graf CCPHP offers four payment options: a single annual payment, semi-annual payments, quarterly payments, or monthly payments. Annual payment carries no processing fee. 5% respectively per installment.

Membership begins on the effective date confirmed by Graf CCPHP after it receives your signed agreement and membership fee. The initial term runs one year and renews automatically for successive one-year periods. Either party may choose not to renew with at least 30 days written notice before the term ends.

Yes, either party may terminate the agreement at any time with 30 days written notice. If a member terminates early, Graf CCPHP refunds the pro-rata portion of any paid membership fee, minus a $200 administrative fee. The company does not charge the administrative fee if it initiates termination.

Dr. Graf designates a covering physician or licensed medical professional to attend to member needs during any planned or unplanned absence. This ensures continuity of care during illness, vacation, or continuing medical education periods. The membership agreement does not affect Dr. Graf's or any covering practitioner's independent clinical judgment.

Yes, Graf CCPHP reserves the right to change the annual membership fee. The company provides at least 30 days written notice before any fee change takes effect.
