Fermata Direct Primary Care

Fermata DPC charges a flat monthly membership fee with no copays or unexpected bills. Adults ages 19 to 64 pay $100 per month. Children ages 2 to 18 pay $50 per month when enrolled with an adult, or $70 per month otherwise. Adults 65 and older pay $150 per month. New households pay a one-time enrollment fee of $200. Families with three or more members receive a 10% discount.

Fermata DPC does not bill insurance companies. The practice operates entirely on a flat monthly membership fee. Dr. Asistores recommends that members carry a high-deductible health plan to cover costs like hospitalization, surgical procedures, or specialist care that fall outside the practice's scope.

Yes. Medicare patients can join Fermata DPC. Upon enrollment, members sign a one-time waiver stating that neither the patient nor Dr. Asistores will bill Medicare directly for services provided by the practice. Medicare still covers medications and lab tests that Dr. Asistores prescribes or orders.

Fermata DPC schedules appointments for 30 to 60 minutes on average. New patient visits run up to 90 minutes. Same-day and next-day appointments are available. The practice keeps patient volume low, so wait times are minimal.

Yes. Dr. Asistores offers virtual visits and after-hours consults by phone, video, text, or email. He also makes house calls when available. Home visits more than 15 miles from the office may carry an additional fee.

Membership covers a wide range of primary care services. These include annual physicals, well-child visits, school and sports physicals, pap smears, acute and urgent care visits, chronic condition management, EKGs, urinalysis, and flu and strep tests. Dr. Asistores also performs cryotherapy, ear wax removal, nebulizer treatments, minor surgical procedures, steroid injections, and behavioral health care. Lab pathology costs for certain procedures fall outside the membership fee.

Prospective patients can call, email, or fill out the enrollment form on the Fermata DPC website. Dr. Asistores also offers a free one-time consultation for anyone who wants to discuss whether the practice fits their health needs. Online enrollment takes about 10 to 15 minutes to complete.

Yes. Members who stay generally healthy still benefit from the relationship with Dr. Asistores. He works with patients proactively to identify risk factors, plan preventive screenings, and set health goals tailored to their life and values. All of this falls under the monthly membership fee with no additional costs.

For emergencies, members should call 911 or go to the nearest emergency department. If hospitalization follows, Dr. Asistores communicates directly with your hospitalist to inform your care plan. When specialist care is needed, he personally refers and coordinates your care with the appropriate provider.

Yes. Fermata DPC accepts children starting at age 2. Dr. Asistores provides well-child visits, school and sports physicals, and acute care for younger patients. Vaccines for children are referred to the health department. Children enrolled with an adult member pay $50 per month.
