Equinox Primary Care uses age-tiered monthly pricing. Patients aged 0-19 pay $90 per month, those aged 20-39 pay $100, those aged 40-59 pay $120, and patients 60 and older pay $140. The monthly fee covers all visits, communications with the practice, and coordination of care.

Yes, Dr. Bliss offers phone, video, and electronic communication appointments. Telehealth services are available as part of the standard membership and allow patients to connect with Dr. Bliss without an in-person visit.

Yes. Dr. Bliss conducts home visits as well as visits to nursing homes and assisted living facilities. These visits may involve additional fees beyond the monthly membership rate.

No. Dr. Bliss's practice is currently full and does not accept additions to the waitlist. Prospective patients should contact the office for updates on future availability.

Equinox requires at least 24 hours' notice for appointment cancellations. Patients who no-show or cancel late three or more times may face scheduling restrictions. Chronic no-shows may result in membership termination.

Equinox provides at least 60 days' written notice before any fee schedule change. The practice limits fee changes to no more than once per year.

Equinox Primary Care has opted out of Medicare. Medicare beneficiaries who join must sign a Medicare Opt-out Agreement before their first appointment. This agreement does not affect their ability to use Medicare benefits with other providers outside of Equinox.
