Monthly membership fees at Elevated Health vary by age. Children ages 0 to 19 pay $30 per month and must enroll with an adult. Adults ages 20 to 45 pay $85 per month, ages 46 to 64 pay $105 per month, and members 65 and older pay $125 per month. A one-time enrollment fee of $75 per person applies, with a family maximum of $150 when enrolling at the same time. Members who pay annually receive an 8 percent discount, effectively getting one month free.

Elevated Health does not accept insurance for monthly membership fees. The practice operates on a direct primary care model, meaning members pay a flat monthly fee directly to the practice. Many members pair their membership with a high-deductible insurance plan or health sharing plan for coverage of hospitalizations and specialist care.

Membership covers unlimited office visits with no copay, general adult physicals, women's healthcare, newborn and pediatric care, wellness education, chronic disease management, minor procedures, and coordination of care with specialists. Members also receive free diagnostic EKG and spirometry testing, plus four skin lesion removals per year at no additional charge.
