At Cottontree Family Practice, two board-certified family physicians serve the Provo, Utah community with deep roots spanning more than three decades. Dr. Richard Holmes and Dr. James Woodmansee lead the practice together, each bringing decades of clinical experience in family medicine. The practice operates on a direct primary care model, collecting a flat monthly retainer and billing no insurance carriers.

The $65 monthly fee covers unlimited office visits, physical exams, routine labs, EKG, X-rays, and procedures. Cottontree serves patients of all ages, from newborns through seniors, and welcomes everyone regardless of health history or background. Clinical services include sports medicine, pediatrics, minor surgery, fracture care, vasectomy, Votiva and Morpheus8 radiofrequency treatments, counseling, and ultrasound. The practice holds preceptorship relationships with the University of Utah PA Program, the University of Arizona, the University of Nevada Touro PA Program, and the BYU School of Nursing. Many patients have brought their children and grandchildren here, building multigenerational relationships with the same physicians.
