Community Direct Care

Monthly membership rates at Community Direct Care vary by age. Children ages 5 to 17 pay $10 per month with at least one parent membership. Adults ages 18 to 44 pay $59 per month. Adults ages 45 to 64 pay $79 per month. Adults ages 65 to 99 pay $99 per month. The practice charges no hidden fees on top of the membership rate.

Community Direct Care operates on a direct pay model. Members pay a flat monthly fee directly to the practice rather than using insurance for primary care visits. Many members carry a high-deductible insurance plan or a health-sharing plan alongside their membership to cover hospitalizations or specialist care.

Community Direct Care passes labs and medications to members at cost. A cholesterol panel costs $6. Common medications often run $4 or less. These prices are well below what most patients pay through insurance billing.

Yes. Members reach Dr. Shuh directly by call, text, or email. The practice prefers text. This direct line replaces the process of calling a front desk, leaving a message, and waiting for a callback.

The practice covers a wide range of primary care needs including annual physicals, diabetes management, hypertension, weight management, and cholesterol screening. Dr. Shuh also addresses anxiety, depression, sleep issues, and substance use support. Urgent care services include DOT physicals, minor laceration repair, flu and strep and COVID screening, skin infections, and sprains and strains.

Yes. Dr. Shuh became a board-certified Addiction Medicine Physician in December 2025 and now accepts addiction medicine patients on a limited basis. Addiction Medicine patients also receive all standard Family Medicine services. Monthly rates for Addiction Medicine patients start at $110 for adults ages 18 to 44 to account for additional time and specialty consultation.

The practice sees patients Tuesday through Friday from 9:30 AM to 4:30 PM and Saturday from 8:00 AM to noon. The office is located at 1850 Eastgate Rd., Suite C, Toledo, OH 43614.

The practice team includes Dalila Saleh, a board-certified nurse practitioner with a background in ICU, emergency, and behavioral health care. Kayla Weaver, a licensed massage therapist with eight years of experience, serves as a BWC- and VA-approved provider for medical and therapeutic massage. Joshua Sexton works as a medical assistant, and Tammy McInerney handles administrative support.
