Membership fees vary by provider and age group. For patients ages 19 to 39, fees start at $95 per month or $1,100 per year. Rates increase for older age groups, with patients 55 and over paying $180 per month or $2,000 per year. The practice offers an annual discount when members pay in full by check. A one-time enrollment fee of $150 per member also applies.

Membership includes unlimited office visits without copays, same-day appointment availability, and up to 60-minute appointments.
