Monthly membership fees at CCC Health vary by age. Children ages 2 to 17 pay $50 per month. Adults ages 18 to 44 pay $60 per month. Adults ages 45 to 64 pay $75 per month. Members 65 and older, and children with no adult on the plan, pay $95 per month. A one-time enrollment fee of $75 applies to all new memberships.

Membership at CCC Health covers sick visits, in-person and telemedicine consultations, school and sports physicals, law enforcement and pre-op physicals, visual acuity screening, color vision testing, PA hearing tests, supplement reviews, and monthly wellness seminars. Members also receive discounted rates on a la carte services not included in the base membership.

Yes. Members continue to use their health insurance for services outside the practice, including specialist visits, hospital care, and prescriptions filled elsewhere. CCC Health does not accept insurance for its own services, and the monthly membership covers primary care directly.

Yes. As of January 1, 2026, members can use HSA funds to pay for their DPC membership fees at CCC Health.

CCC Health offers same or next-day sick visits, including on weekends. Members also have 24/7 access to their physician by phone, email, and text for urgent needs outside of office hours.

Appointments typically run 30 to 60 minutes depending on the patient's needs. The practice schedules adequate time for each visit so physicians can address health concerns thoroughly.

Yes. CCC Health offers telemedicine consultations for members. Members can schedule virtual visits through the practice, and telemedicine is included in the membership at no additional cost.

CCC Health performs a wide range of in-office procedures, including EKGs, pulmonary function tests, nebulizer treatments, suturing, dermascopy, incision and drainage, circumcision, skin tag removal, body composition analysis, and ImPACT concussion baseline testing. Members receive discounted rates on procedures not included in the base membership.

Yes. Light The Path Physical Therapy operates on-site at CCC Health and serves members and non-members alike. Ann R. Ed. offers on-site mental health sessions, including "walk and talk" therapy focused on intentional living. Both services operate independently and are not covered under the CCC Health membership.

Both Dr. Greg Wheeler and Dr. Chaminie Wheeler have opted out of Medicare. Patients with Medicare sign a private contract acknowledging that neither party will submit claims to Medicare for services at CCC Health. Patients may still use Medicare freely for all other healthcare services outside the practice.
