The immigration physical exam and Form I-693 are not covered by insurance, and the $400 fee is due at the time of service. However, PPO insurance may cover eligible vaccines and lab work ordered as part of the exam. Patients with Kaiser insurance can receive required vaccines or lab work through Kaiser and send the records to Careplus MD.
